Resistance Training for Longevity: The 2×/Week Minimum Effective Dose

Cardio gets the headlines. But resistance training has the strongest mortality data per minute invested. A 2022 meta-analysis of 16 studies (N>480,000) found 30–60 min/week of resistance training → 15–20% lower all-cause mortality, independent of aerobic exercise. The dose-response plateaus at ~60 min/week. Two 30-min sessions. That’s the minimum effective dose.

The Mortality Evidence

Study Population Resistance Training Dose All-Cause Mortality Reduction
Momma et al. (2022) BJSM 16 studies, 480k+ adults 30–60 min/week 15–20% (peak at 60 min)
Gorzelitz et al. (2022) JAMA Netw Open 99k US adults 1–2×/week 22% vs none
Kamada et al. (2017) Med Sci Sports Exerc 28k women ≥2×/week 28% cardiovascular mortality

Key: Benefits plateau at ~60 min/week. More ≠ better for longevity (though more = better for hypertrophy).

The 2×/Week Program (Full Body, 30 Min)

Session A Sets × Reps Session B Sets × Reps
Goblet Squat 3 × 8–12 Romanian Deadlift 3 × 8–12
Push-Up / Bench Press 3 × 8–12 Pull-Up / Row 3 × 8–12
Overhead Press 3 × 8–12 Farmer Carry 3 × 30 sec
Plank 3 × 30–60 sec Side Plank 3 × 30 sec/side

Progression: Add 1 rep/set weekly until top of range → add weight → drop reps to 8 → rebuild.

Why This Works (Mechanisms)

Mechanism Mortality Link
Muscle mass → metabolic reserve Sarcopenia → insulin resistance, falls, frailty
Strength → functional independence Chair rise, stair climb, carrying groceries
Bone density → fracture prevention Hip fracture 1-year mortality: 20–30%
Myokines (IL-6, irisin) Anti-inflammatory, insulin-sensitizing
Neural drive → fall prevention Rate of force development predicts falls

Equipment Hierarchy (Best to Good Enough)

Tier Equipment Cost Space
1. Adjustable dumbbells + bench $300–500 4×6 ft Best versatility
2. Kettlebells (2–3 sizes) $100–200 3×3 ft Great for swings, carries
3. Resistance bands + door anchor $30–50 Portable Good for travel/start
4. Bodyweight + backpack $0 None Progress via leverage

Minimum: One pair of adjustable dumbbells. Covers 90% of this program.

Recovery Rules (Non-Negotiable)

Rule Why
48h between sessions Muscle protein synthesis peaks 24–48h
Protein: 1.6–2.2 g/kg/day Morton et al. (2018): max MPS at 1.6 g/kg
Sleep 7–9h GH/testosterone peak in deep sleep
Deload every 6th week Reduce volume 50% → prevents overreaching

The Counterpoint: When You Need More

Goal Additional Work
Hypertrophy (muscle size) 10–20 sets/muscle/week → add 2nd session
Athletic power Add plyometrics, Olympic lifts
Rehab / prehab Specific isolation, tempo work
Aesthetic detail Accessory isolation (arms, calves, side delts)

The Takeaway

Two 30-min sessions per week. Compound movements. Progressive overload. Protein + sleep. That’s 15–20% lower all-cause mortality for 1 hour/week. No gym membership required. No complexity. The highest ROI intervention in health. Start this week.
